Undertaking
6/2/2026
On January 27, 2026, the Inquiries, Complaints and Reports Committee (ICRC) identified concerns with Zalak Patel’s practice in the areas of assessment, diagnosis and treatment in the area of pelvic health, and documentation. The ICRC provided Zalak Patel with the opportunity to improve their understanding of these areas by reviewing relevant standards/resources, completing learning activities and participating in practice enhancement coaching. Zalak Patel agreed to complete this learning through signing a voluntary Undertaking.
An Undertaking is a commitment by a registrant of the College to address concerns about certain areas of practice and/or conduct that have been identified by the ICRC as areas requiring improvement. An Undertaking does not represent a finding of professional misconduct or incompetence, and it is not disciplinary in nature. As required by law, the College must publish notice of signed Undertakings on the public register. This notice will be removed from the public register if and when the terms of the signed Undertaking have been successfully completed by the Registrant.

Caution
12/7/2021
On November 9, 2021, the Inquiries, Complaints and Reports Committee (“the Committee,” “ICRC”) made a decision regarding information it obtained about the conduct of Zalak Patel for failing to meet their regulatory obligations and complete the annual Professional Issues Self-Assessment (PISA). The Committee decided that the concerns about Zalak Patel’s conduct were serious enough to require that Zalak Patel attend before the Committee to be cautioned.
Since 2017, as required by law, the College has been posting cautions on the public register. A caution is a decision made by the ICRC when a concern raised in a complaint or in a formal investigation is sufficient to require the physiotherapist to attend before a panel of the ICRC to receive feedback from the panel on how to prevent similar concerns from arising in the future. A caution does not involve a finding of wrongdoing, professional misconduct or incompetence and is not disciplinary in nature.
